• All patient will be asked to show current health insurance card and their driver’s license at each visit with their provider. Co-pay, deductible, and/or co-insurance will be due at time of service. We no longer accept cash or check. Card ONLY.

• Effective since 2023 the Medicare Part B deductible is $226.

  • Medicare patients will be responsible for 20% of the office visit charges after your deductible is met.

  • Most secondary insurance will pay the 20% office visit charge.  Patient is responsible for any unpaid charges

• Telehealth visits are NO LONGER BEING OFFERED at this time.
